Unit 1: Team Project Guidelines

Team Formation

The instructor will assemble project work teams, which will consist of a Project Manager (PM) and team members. The assigned PM will be the team leader and will be responsible for planning, organizing, directing, controlling, and coordinating the effort of the team to ensure that the assigned project and its’ deliverables are successfully completed by the established deadline. Team sizes may vary based on courses enrollment and the nature and scope of the projects to be completed; however, the total team will have no fewer than three (3) and no more than four (4). The goal is to keep the team sizes small enough that each member will have an equal opportunity to make a substantive contribution to the overall success of the team in terms of meeting its goals and objectives.

Important Notice: It is at the sole discretion of the instructor to change team members with or without advance notice for cause or for no reason at all. In addition, the instructor has the right to change the parameters of the team projects, including, but not limited to: moving up or pushing back due dates, changing deliverable expectations, or request progress reports.

Team Meetings – Units 2-7

Online Students

Each team will utilize Zoom to conduct and record their team meeting. Failure to record any of these mandatory meetings will adversely affect the both PM and team members’ grade, which will be assigned by the faculty at the end of unit 7. The meeting MUST be conducted at some point during the unit, which runs from Monday through Sunday. This will allow ample flexibility to accommodate work schedules and the geographic location of the participants (domestic or international). It is expected that a typical meeting will last 45 minutes to an hour depending on the level of group and individual preparation; however, there is no artificial limitations on the length of time the team can participate in the meeting. Zoom facilitates the sharing of one’s computer screen, which makes it possible to use these meetings as actual work session. There is no long distance charge associated with using Zoom.

  • The Team Project includes two parts.
    • In the first part, conduct your research on the project topic – a Patient Centered Medical Home (PCMH) model, following the project questions.
    • In the second part, examine CareFirst Patient Centered Medical Home of Maryland, its internal and external environment, and develop a proposal for the key decision makers with specific recommendations.

Part 1

  1. Explain the concepts behind the Patient Centered Medical Home (PCMH) model, also known as the medical home model.
  2. Conduct systematic analysis of this model, breaking it down to components.
    • What are the PCMH reimbursement models, incentives, economic, quality of care, legislative, staffing, and technology considerations?
    • What approaches are working better? What is not working?
  3. Look at it from a critical point of view.
    • What would you recommend, how to implement the PCMH model more efficiently?

Team Project Deliverables

There are no page limitation associating with this deliverable; however, it should explicitly address all required components of this assignment. It is expected that the quality of the product will be consistent with that expected of industry professionals. Special attention should be given to the quality and accuracy of the content; the layout of the content; and ensuring the topics flow logically throughout the document. The document should be prepared as a practical business deliverable (single-spaced). The title page should appear as a cover of a business deliverable. The in-text citations and the list of references must adhere to the APA writing style (7th ed.).

To earn the maximum number of points, the content should reflect graduate-level execution with clear evidence of critical thinking, synthesis of relevant information from credible sources, and clear mastery of the concepts necessary to successfully execute each component of the assignment. The Team Project Deliverable should be prepared as a Microsoft Word document and uploaded in the appropriate Unit 8 assignment page by Friday, 11:59 p.m. (CT) of Unit 8.
